摘要:
目的 探討多層螺旋CT增強掃描在肺癌鑒別診斷的價值。方法 選取2017年12月~2018年12月我院收治的90例肺部占位疾病患者,均行多層螺旋CT平掃、增強掃描,比較不同病理類型肺內病灶的平掃/增強掃描的CT值及強化增值、不同肺癌病理類型CT凈增強值,及多層螺旋CT平掃、增強掃描診斷肺癌的準確性、敏感性及特異性。結果 肺癌、肺結核球、炎性假瘤平掃CT值比較,差異無統計意義(P>0.05);肺癌增強掃描CT值高于肺結核球,低于炎性假瘤,差異有統計學意義(P<0.05);肺癌強化增值高于肺結核球,低于炎性假瘤(P<0.05)。鱗癌CT凈增強值高于腺癌、小細胞癌,差異有統計學意義(P<0.05);腺癌CT凈增強值高于小細胞癌,差異有統計學意義(P<0.05)。增強掃描診斷肺癌的準確性為88.89%、敏感性為92.31%、特異性為84.21%,均高于平掃的67.78%、69.23%、65.79%,差異有統計學意義(P<0.05)。結論 多層螺旋CT增強掃描可以幫助鑒別肺部占位疾病及不同病理類型肺癌,且診斷的準確性、敏感性及特異性均較高。
Abstract:
Objective To investigate the value of multi-slice spiral CT enhanced scan in differential diagnosis of lung cancer. Methods A total of 90 patients with pulmonary space disease admitted to our hospital from December 2017 to December 2018 were enrolled in this study. All patients underwent multi-slice spiral CT scan and enhanced scan to compare the plain/enhanced scans of different pathological types of lung lesions. CT value and enhanced value-added, CT enhancement of different lung cancer pathological types, and accuracy, sensitivity and specificity of multi-slice spiral CT plain scan and enhanced scan for diagnosis of lung cancer. Results There was no statistically significant difference in the CT values of lung cancer, tuberculoma, and inflammatory pseudotumor (P>0.05). The CT value of lung cancer enhanced scan was higher than that of tuberculosis, which was lower than that of inflammatory pseudotumor,the difference was statistically significant (P<0.05); the enhanced value of lung cancer was higher than that of tuberculosis, which was lower than that of inflammatory pseudotumor (P<0.05). The net enhancement of CT in squamous cell carcinoma was higher than that in adenocarcinoma and small cell carcinoma,the difference was statistically significant (P<0.05); the net enhancement of CT in adenocarcinoma was higher than that in small cell carcinoma,the difference was statistically significant (P<0.05). The accuracy of enhanced scan for diagnosis of lung cancer was 88.89%, sensitivity was 92.31%, and specificity was 84.21%, which were higher than 67.78%, 69.23%, and 65.79% of the plain scan,the difference was statistically significant (P<0.05). Conclusion Multi-slice spiral CT enhanced scan can help identify lung occupying diseases and different pathological types of lung cancer, and the accuracy, sensitivity and specificity of diagnosis are high.
