摘要:
目的 探究高血壓家族史患者正常高值血壓對頸動脈內膜中層厚度(IMT)及血清代謝指標的影響。方法 回顧性分析2019年5月~7月我院體檢的300名健康者的體檢資料,依據血壓值及是否存在高血壓家族史分為血壓值正常且無高血壓家族史的A組(110名)、正常高值血壓且無高血壓家族史的B組(90名)、正常高值血壓且伴高血壓家族史的C組(100名),比較三組血清代謝指標、[膽固醇(CHO)、低密度脂蛋白(LDLC)、尿酸(UA)、同型半胱氨酸(Hcy)]、頸動脈IMT值,分析正常高值血壓人群頸動脈IMT影響因素。結果 C組CHO、LDLC、UA、Hcy、IMT值高于A、B組,且B組CHO、LDLC、UA、Hcy、IMT值高于A組,差異有統計學意義(P<0.05);ROC曲線分析顯示,IMT值評估正常高值血壓人群伴高血壓家族史的曲線下面積為0.821,臨界值為0.91 mm;多因素Logistic回歸分析顯示,伴高血壓家族史是正常高值血壓人群頸動脈IMT增厚的危險因素(P<0.05)。結論 伴高血壓家族史患者正常高值血壓有增厚IMT及增加血清代謝指標異常的風險,該類人群在健康體檢時需重點篩查,以提高高血壓疾病的防治。
Abstract:
Objective To investigate the effect of normal high blood pressure on carotid intima-media thickness (IMT) and serum metabolic indexes in patients with family history of hypertension.Methods The medical examination data of 300 healthy people undergoing physical examination in our hospital from May to July 2019 were retrospectively analyzed.According to blood pressure value and family history of hypertension, they were divided into group A (n=110) with normal blood pressure and no family history of hypertension, group B (n=90) without family history of hypertension, group C (n=100) with normal high blood pressure and family history of hypertension, compared serum metabolic indicators [cholesterol (CHO), low density lipoprotein (LDLC), uric acid (UA), homocysteine (Hcy)] and carotid IMT values of the three groups, and analyzed necks of people with normal high blood pressure factors influencing arterial IMT.Results The values of CHO, LDLC, UA, Hcy, and IMT in group C were higher than those in group A and B, and the values of CHO, LDLC, UA, Hcy, and IMT in group B were higher than those in group A,the differences were statistically significant (P<0.05); ROC curve The analysis showed that the area under the curve for assessing the family history of hypertension with IMT in normal high blood pressure was 0.821, and the cutoff was 0.91 mm.Multivariate Logistic regression analysis showed that a family history with hypertension was a risk factor for carotid IMT thickening in normal high blood pressure patients (P<0.05).Conclusion Patients with a family history of hypertension have a risk of thickening IMT and increasing abnormal serum metabolic indexes. This type of population needs to be screened during physical examination to improve the prevention and treatment of hypertension.
